Proper Format for a Power of Attorney Letter

Generally, a good power of attorney letter has the following structure:

1. Letterhead (Optional)

If from an institution/agency, use official letterhead.

2. Letter Title

“POWER OF ATTORNEY” written in bold at the top center.

3. Grantor’s Identity

Contains:

• Full name

• Place and date of birth

• Occupation

• Complete address

• ID card number (if required)

4. Recipient’s Identity

Contains complete data as above.

5. Content/Authority Granted

Explains specifically what authority is given to the recipient.

6. Substitution Rights (Optional)

States whether the recipient can delegate authority to others.

7. Date and Place of Letter Creation

8. Signatures

Signatures of both parties (grantor and recipient).

9. Stamp Duty

For some official purposes, Rp 10,000 stamp duty is required.
